On May 28, 2015, a truck driver entered the scale platform from the opposite direction of its designed use. The scale platform was designed with a walking surface on the driver's side and a guardrail on the passenger side. The driver exited his truck and stepped onto the top rail of the guardrail system. It was raining at the time. The driver slipped and fell to the ground. The driver experience a compound fracture to his left arm. The driver was taken to the hospital, treated, and release

Employees were manually aligning 6"x6"x9' stainless steel beams weighing approximately 300 lbs on the tank stands. The injured employee and another employee were aligning beams in the same sequence on the same line. The supervisors were making the final adjustments when the employee attempted to help and his right little finger was pinched between the beam and the beam that was already in place. The employee was wearing gloves at the time of the incident. This resulted in a partial amputation
